In December 2012, the Ministry of Health and Long-Term Care released its Action Plan for Health Care. Complementing the release of the Action Plan in 2012, and in an effort to highlight the need for more patient-focused, coordinated care for the highest cost and most complex users of health care resources, the Ministry launched the province-wide Health Links initiative.
To further advance the coordination of care for patients with complex care needs following the Health Links approach to coordinated care planning within the seven Central East LHIN Sub-regions. The purpose of the Coordinated Care Planning Working Group is to develop, implement and sustain business processes for the advancement of coordinated care planning within each Sub-region in alignment with the Health Links Maturity Journey.

The Central East LHIN Coordinated Care Planning eLearning Module was developed to support the education, understanding and on-boarding of Central East LHIN health service providers to coordinated care planning practices. To complete the Coordinated Care Planning eLearning Module, please see HERE. Individuals who successfully complete part 1 and 2 of the module are eligible to receive the Central East LHIN Coordinated Care Planning certificate.
