Health Links

Health Links is a local health care network consisting of patients, caregivers, health care providers, and community support agencies that are committed to working better together to effectively identify every patient with complex health care needs and improve their health outcomes through Coordinated Care Planning. Health Link networks have shared in the development and implementation of Coordinated Care Plans (CCPs) since the launch of the initiative in December 2012.

The Ministry of Health’s funding for Health Links has come to an end as of March 31, 2020. However, providing integrated, quality care for the most complex patients is aligned with the Ministry of Health’s directions for the health care system.

Home and Community Care will continue to play a role to support and sustain integrated care planning for Home and Community Care patients with complex care needs. Further resources to support ongoing Coordinated Care Planning and CHRIS/HPG can be found here.
