2012-13 Year in Review

2013-16 Integrated Health Service Plan
The Central East LHIN is proud to release its third Integrated Health Service Plan (IHSP), a 45-page document that sets out a shared “Community First” goal for the local health care system. Outcome focused, clear and measurable, the Central East LHIN’s 2013-16 IHSP sets out a bold, but realistic, road map for health care improvement for our region. The Value of LHINs
LHINs are the only organizations in Ontario that bring together health care partners from the following sectors – hospitals, community care, community support services, community mental health and addictions, community health centres and long-term care – to develop innovative, collaborative solutions leading to more timely access to high quality services for the residents of Ontario and Central East LHIN communities. By supporting these important partnerships, LHINs are ensuring that Ontarians have access to an effective and efficient health care system that delivers improved health care results and a better patient experience.

Home First
Home First is a significant shift in health care thinking. When a person enters a hospital with an acute episode, every effort is made to ensure adequate resources are in place to support the person to ultimately go home on discharge. Only when returning home with care is not possible or safe to do so, are other options considered. Essentially, Home First is about providing the right care, in the right place, at the right time and for the right cost.
