Chronic conditions are long-term health issues that impact a person’s life for many years. Examples of chronic conditions include heart disease, stroke, chronic kidney disease, respiratory disease, diabetes and arthritis.

People with chronic conditions spend only an estimated average of 12 hours with health service providers in the course of an entire year – and the remaining 364+ days in their communities, with their families, and in their homes and workplaces. 

How people manage their symptoms, medications, eating and physical activity during those 364+ days (their “Self-Management”) has a powerful impact on people’s well-being and health outcomes.

The CE LHIN’s Self-Management Training Priority Project, launched in April 2008, introduces a consistent, co-ordinated and sustainable Chronic Disease Self-Management training model across Central East LHIN. 

The Living a Healthy Life with Chronic Conditions six-week self-management workshop empowers people to develop new tools and skills to break the cycle of symptoms that can result from chronic conditions.  It is widely considered a best practice in chronic disease self-management and has been adopted in many parts of Canada and around the world.

This LHIN-funded initiative provides self-management training to 1,400 consumers & caregivers, and numerous health service providers, in its first two years.   As of  January 2009, already over 350 consumers and caregivers have attended self-management workshops.

The Central East Community Care Access Centre (CECCAC) has taken the role of Lead Program Sponsor Agency and holds the Chronic Disease Self-Management program license from Stanford University.

THE PROJECT TEAM

The Self Management Project Team consists of health service providers, consumers, and caregivers from all areas of the Central East LHIN who are working together to guide and implement the Self-Management Training Priority Project.  They meet in person and by teleconference, every 2-3 weeks from May 2008 through March 2010.
