CCIM – OCAN & IAR
|
The Central East LHIN and Community Care Information Management (CCIM) are working together to support health service providers from across the Central East region to implement a number of standardized tools that will help to provide improved services and measure the improvements in the quality of care in the Central East LHIN. Community Care Information Management (CCIM) plays an integral role in improving the health care system by enhancing access to information for community-based care providers to deliver the right health service, at the right time.

Common Credentialing
|
Purpose: Create a credible common credentialing process across the LHIN that assures the public, boards and the Central East LHIN that due diligence has been conducted.
The credentialing framework is intended to include the re-credentialing component of the re-appointment process.
Business Value:
- Assure the public, hospital board and Central East LHIN that processes are in place to protect the public (i.e. adoption of leading practices for credentialing).
- Build operational efficiencies with the process through common standards and processes across the LHIN (i.e. reduction of administrative duplication for hospitals and physicians who chose to work at multiple sites).
- Facilitate physician mobility across the LHIN in crisis events (i.e. pandemic outbreaks) and for those physicians who choose to work at multiple sites.

PLANNING

Common Network, WAN and Security Management
|
Purpose: Consolidated Network Management, and a strenghtening of expert staff to centralize security standards, risk management, security monitoring, incident response and breech management.
Business Value Reduced duplication, common directives, specialization, improved monitoring, best practices, and better foundations to shared projects

Common Procurement & Optimization
|
Purpose: Common procurement and standardization of IS/IT equipment and solutions (e.g. WAN bandwidth, printers, etc).
Business Value: Reduced procurement costs, standardization of devices resulting in reduced user issues, shorter issue resolution times, and a provincial approach.

Connecting GTA
|
Purpose: Connecting GTA is a project with the 5 GTA LHINS to “integrate electronic patient information from across the care continuum and make it available at the point-of-care to improve the patient and clinician experience”
Business Value:
- ConnectingGTA will provide better access to information for up to 700+ health care organizations resulting in better care delivery for 5.8 million GTA residents.
- Patients will receive improved, timelier and more coordinated care
- Clinicians will be able to initiate timely treatment, improve productivity and better collaborate with peers
- Health care organizations will be able to maximize their investments and resources and improve efficiency
- Our health care system will see improvements in care coordination and capacity

Data Centre Consolidation
|
Purpose: The data centre consolidation to foster and support eHealth growth; mitigate operational risk; improve existing service levels; and position the participating hospitals for future cost reduction, and operational enhancements such as application consolidation.
Business Value:
- Reduce the risk associated with current data centres
- Enable disaster recovery and business continuity capabilities
- Improve infrastructure reliability enabling sharing of hospital applications resulting in decrease in information management (IM) and information technology costs, and the improvement of flow of clinical information between hospitals
- Improve service level performance
- Provide a platform to build standardization of clinical best practices

Diabetes Indicators
|
Purpose: A Pilot project in conjunction with the Diabetic Educators in the LHIN looks at the trends in 6 biophysical indicators for patients in the LHIN
Business Value:
- Use information technology and information management to support people and families and improve the health and safety of the population in the LHIN and beyond
- Streamline business processes to facilitate efficiency
- Support providers in delivering care

Eclipse
|
Purpose: Support and encourage the use of the ECLIPSE Project Portfolio Management software tool in the CE LHIN.

- Improve project management of multi-resource, cross-organization projects

eHealth Community Consultation
|
Purpose:
- To review and accept the Central East LHIN eHealth Vision, and provide recommendations to the eHealth Steering Committee on any changes or additions to the eHealth Strategic Plan.
- To identify, prioritize and provide project planning and actions to e-Health initiatives that align with the Central East LHIN IHSP and needs of the community agencies and provincial strategy.
- Develop knowledge and influence on e-Health within the smaller community agencies and Physician groups through key stakeholders and champions to promote and progress the use of e-Health (both the Central East LHIN initiatives and the Provincial initiatives).
- Use the resultant needs and recommendations in conjunction with current plans to determine updated strategies and tactics toward implementation
- Determine the process to integrate community agency representation into the e-Health Steering Committee upon completion of the Task Group.

ER Kiosks
|
Purpose:

|

|
Purpose: To provide a collaboration platform to support and offer a centralized environment for regional initiatives and committee activities.

MVLS – Hospital 6 year ELA
|
Purpose: Central East LHIN assisted in the coordination as the Central East LHIN hospitals collaborated on a group approach under a master contract agreement with Microsoft for licensing volume pricing.
Business Value:
- Hold costs on Microsoft Desktop and Service licenses and product at current prices for 6 years while Healthcare pricing is moved upwards to align with Government pricing and away from Education pricing (approx 30% increase).

Resource Matching and Referral (RM&R)

|

Standardized Order Sets
|
Purpose: The procurement and implementation of an automated Order Set product for the Central East LHIN hospitals to replace the current (monthly) contract with OSOS (Open Source Order Sets) to provide a standard and cost effective method of electronic order sets.

Surgical Utilization Booking Management Integration Tool (SUBMIT)
|
Purpose: To provide a regional web-based solution in order to improve patient Wait List management, and Wait Times reporting for surgeons and hospitals in the Central East LHIN.
Business Value:
- Offer a web-based solution to the surgeons to electronically manage their wait lists; and provide automated rule-based data to the provincial WTIS without data duplication.
- Connect the surgeon’s wait list management, OR booking, pre-screening clinics and admissions areas to provide a real-time and transparent view of patient progress.
- Provide electronic transfer of patient booking packages and supporting documentation.
- Provide access for surgeons to all their OR bookings at multiple privileged sites.
- Ability to provide aggregate data (viewing and reporting) to improve the data quality and reporting of Wait Times at a regional level.

Timely Discharge Information System (TDIS)
|
Purpose: To ensure family doctors and other community physicians received information concerning a patient’s hospital stay within 72 hours of transcription.
Business Value:
- Enabling continuum of patient care in ensuring their physicians have their most recent discharge data and reports.
- Successfully sending data to physician’s EHRs, eliminating time-consuming phone calls, faxes and printed results.
- Significantly reduced the workload on community physician staff who previously had to scan all this paper into the system and insert it into the correct location within the patient’s chart, then shred the paper reports.
- Reports are automatically retrieved by community physician EMR in near real time and automatically inserted into the correct location within the patient’s chart.

|

Utilization Management System
|
Purpose: Procure a utilization management tool(s) to facilitate a standardized, achievable approach to patient disposition/designation and the use of beds by interested hospitals, and the Central East LHIN. The tool(s) will enable hospitals and the LHIN to manage and report on the use of beds in the system.

VM Ware ELA
|
Purpose: VMWare is used to virtualize the systems onto less hardware / equipment and all CE hospitals have the VMware virtualization product(s) in some form. VMWare approached IM/IT Advisory Group to develop an Enterprise License Agreement for all the CE LHIN hospitals (except Campbellford) to realize cost and administration efficiencies.
Business Value:
- To determine the efficiencies / cost benefits of the agreement as opposed to stand alone
- Cost evaluation has shown a 31% potential cost savings on product currently owned and identified as need for the next 3 years the hospitals collectively.
